Both LISTs use the same list item tagS within them. Be mindful that some tags are naturally ONLY within other 'container' tags. AND: Last open tag of a series MUST be the 1st one closed; second-to-the-last, the 2nd one that has its closing tag, ETC.

You only need to use form tags when you want information submitted and stored, as might well be the case with tests or questionnaires.

NOTE: Regarding this form-and-questionnaire builder: THIS DOES NOT BUILD SIMPLE QUESTIONNAIRE ITEMS but rather auto-builds a table-formated / horizontal-rule-separated set of items. Before selecting any sort of question, you MUST Click "New Question." Whether you click "End Question" (after making each particular question) is optional - if you want a horizontal rule below the question, then click "End Question". Everything else must be done in a reasonable, ordered way, and with things that are paired always done in pairs at the approp. time.
